This Patient Disclosure Agreement ("Agreement") is a legally binding contract between you, the patient
("Patient"), and DocMobil Telemedicine Platform ("DocMobil"). By using the DocMobil telemedicine platform,
you agree to the terms and conditions outlined in this Agreement. Please read this document carefully before
using the platform.
1.1 DocMobil provides telemedicine services that allow Patients to communicate remotely with healthcare providers for medical consultations, diagnoses, treatment plans, and other medical services through secure video, audio, and messaging functionalities.
1.2 Telemedicine services are provided by licensed healthcare professionals authorized to practice in the Patient's jurisdiction, subject to applicable laws and regulations.
1.3 While telemedicine consultations can be convenient, they may not replace in-person medical evaluations in all cases. The healthcare provider will determine if a telemedicine visit is appropriate for the Patient's specific medical condition.
2.1 DocMobil is committed to safeguarding the privacy and security of Patient information. All data collected during telemedicine visits is protected in accordance with applicable laws and DocMobil's privacy policy.
2.2 Patients are responsible for using secure internet connections and private locations during telemedicine consultations to ensure the confidentiality of sensitive medical information.
2.3 DocMobil uses industry-standard encryption and security protocols to protect data transmitted through the platform. However, Patients acknowledge that no online service is entirely free from security risks, and DocMobil shall not be liable for any unauthorized access or data breach that is beyond its reasonable control.
3.1 Telemedicine consultations provide medical advice and services solely based on the information provided by the Patient during the virtual visit. The healthcare provider may not have access to complete medical records, physical examination, or diagnostic tests, which could limit the accuracy of the diagnosis and treatment recommendations.
3.2 Patients must provide accurate and complete medical history and information during telemedicine consultations to enable the healthcare provider to make informed decisions.
3.3 In cases where a physical examination is necessary, the healthcare provider may recommend an in-person visit or referral to a local healthcare facility.
3.4 In emergencies or urgent medical situations, Patients should seek immediate medical attention at a nearby hospital or contact local emergency services.
4.1 Patients must have access to a reliable internet connection and compatible devices (e.g., smartphone, tablet, or computer) to use the DocMobil telemedicine platform.
4.2 DocMobil is not responsible for any technical issues on the Patient's end that may affect the quality or availability of telemedicine services.
5.1 Patients are responsible for any fees associated with telemedicine consultations, which will be communicated during the scheduling process.
5.2 The availability of insurance coverage for telemedicine services may vary depending on the Patient's insurance policy. Patients are encouraged to verify coverage with their insurance providers.
6.1 DocMobil and its healthcare providers shall not be liable for any damages, claims, or losses arising from the use of the telemedicine platform, except where such liability cannot be excluded under applicable laws.
7.1 DocMobil reserves the right to amend this Agreement at any time. Patients will be notified of any changes, and continued use of the platform constitutes acceptance of the revised terms.
7.2 Patients may terminate their use of the DocMobil telemedicine platform at any time by discontinuing the use of the services.
By using the DocMobil telemedicine platform, you acknowledge that you have read, understood, and agree to be bound by this Patient Disclosure Agreement.
